Initial In-Home Evaluation Form

The first step before any subbing appointment can take place, is the Initial In-home Evaluation Form and payment. Prior to booking a sub, I must meet with you in your child care space to review your policies and procedures. It is important that I be familiar with the space prior to subbing and know where to find essentials such as First Aid equipment, files, and supplies. Anything that licensing would need access to, I need to access to as well. I would like to meet any children who may be in care.

I will need verification that the space meets all of the health, safety and supervision guidelines for licensed family child care. This evaluation is strictly confidential and unless I witness an egregious violation as a mandated reporter, will be kept between us.

I may however decline service if I see multiple or major violations.

At this time, we will also review my contract and policies. If we both agree to proceed, you will be given training records and background check certification to retain for your records. Once both parties have agreed to proceed, licensing will link the subs background check and fingerprinting to your license.

This is an annual fee for me to complete the Initial In-home Evaluation and transfer my credentials to your license.

This includes:

1. Evaluation and daycare space inspection

2. Sub’s credentials given to license holder if applicable

3. Signing of contracts / subcontractor agreement if applicable

If I decline service based on licensing infractions or health factors, I will inform you of the necessary corrections needed in order to hire one of our subs. If you make those corrections, we can return for a follow up evaluation. We will only return once. If compliance is not met a second time, we will decline the service agreement.
